Free jewelry appraisal form template
A one-page appraisal form with room for everything an insurer or a client will ask about: the piece, the stones, the photo, the kind of value and who signed it. Type your business details, print it, and fill it in by hand.
What it covers: who the appraisal is for and why, the type of value, a full item description, metal and weight, center and side stones with grading and lab report details, a photo box, the appraised value, your limiting conditions, and the appraiser's name, credentials and signature.

Say which kind of value it is
The same ring can carry very different numbers depending on the question the appraisal answers.
- Insurance replacement: what it would cost to replace the piece with one of like kind and quality. Insurers usually ask for this.
- Fair market value: the price a willing buyer and a willing seller would agree on. Estates and charitable gifts usually need this one.
- Proof of value: a record that the piece exists and what it is worth, for example for a customer's own files.
- Check one box, and do not mix value types on the same document.

Doing this in Flawless POS
In Flawless, appraisals start from the item or the customer's own piece, with photos and stone details (GIA reports sync to the item), an appraisal type, and a PDF your appraiser signs on their phone. The appraisal stays on the customer's record. See jewelry appraisal software.
